New Student Registration
- You will be required to fill out an Affidavit of Residency, Registration Form (OLR Household Form), and provide Proof of Residency.
- Provide a current utility bill or mortgage bill in parent’s name (NO PHONE BILLS) dated within 60 days of enrollment.
- If you live with another family in their home, please contact Zoe Hernandez (Zoe.Hernandez@sweetwaterschools.org) regarding any additional forms and proof that are required.
- If you are not the parent or legal guardian, you will need an approved Caregiver Form. Please contact Zoe Hernandez (Zoe.Hernandez@sweetwaterschools.org) regarding that form.
- Bring in shot record or a copy of immunization records.
- Provide a check-out sheet with current grades from last school if the student is enroling after the start of the school year.
- Provide transcripts from ALL previous schools.
- Provide a copy of current IEP, if applicable.
- In order for you to receive an Activation Code, your student MUST be enrolled at CPH.
- Once student is enrolled you will receive an Activation Code to create an Infinite Campus Parent Portal. Once your Portal Account is created, you can complete Online Registration.
